Exercise physiology for chronic conditions, with Medicare

Evidence-based exercise for diabetes, heart health, arthritis and long-term pain, in Williamstown. Medicare rebates available with a GP care plan.

Exercise is part of the treatment

For many long-term conditions, the right exercise is one of the most effective treatments there is. It helps control blood sugar and blood pressure, eases arthritis and back pain, and builds the fitness and strength that make daily life easier. An Accredited Exercise Physiologist is trained to prescribe it safely around your condition and medications.

Conditions we work with

  • Type 2 diabetes and pre-diabetes
  • Heart disease, high blood pressure and cardiac rehab
  • Knee and hip osteoarthritis, and joint replacements
  • Chronic low back pain and other long-term pain
  • Osteoporosis and osteopenia
  • Weight and metabolic health

How Medicare works for exercise physiology

If you have a chronic condition, your GP can put you on a chronic condition management plan (often still called a care plan or CDM plan) and refer you to an exercise physiologist. That gives you a Medicare rebate for up to 5 allied health visits a year, shared across the allied health services you use.

Your initial assessment can be bulk-billed with a GP care plan. After your assessment we send your GP a report, so everyone is working from the same page.

To get started, ask your GP about a care plan at your next appointment and mention Moveify in Williamstown. If you are not sure whether you are eligible, call us and we will help you work it out.

Other ways to fund your sessions

  • Private health insurance: most funds cover exercise physiology under extras. Check your level of cover with your fund.
  • DVA: eligible veterans can be referred by their GP.
  • NDIS: we see self-managed and plan-managed participants.

How it works

  • Initial assessment (60 minutes). We talk through your health history and goals, then test your strength, balance and mobility so we have a starting point to measure against.
  • A plan built around you. Every exercise is chosen for your body, your conditions and what you want to get back to.
  • Supervised sessions in the clinic. Your exercise physiologist is with you, adjusting as you go, and progresses the program at a pace your body can handle. Your home exercises sit in the Moveify app.
  • Retesting. We repeat your tests so you can see what has changed, in numbers, not guesses.
